Paying for senior living in Middle Tennessee
Most families discover how the bill is structured on the tour, when it's hardest to think clearly about it. Here is the structure ahead of time — what the advertised number contains, what gets added, and how the two payment models used in this market actually differ.
The anatomy of the monthly bill
The advertised figure is a starting rate: the smallest unit at the lowest care level. Around it sit four charges that determine what a family actually pays:
- The community fee
- A one-time move-in charge, commonly one month's rent or more. Some communities waive it in slow seasons — it is negotiable more often than the rent is.
- Care-level charges
- Added after a nursing assessment, in tiers or points. This is the number most likely to surprise you, and the one to ask about before the tour: "what does your highest care tier add to the base rate?"
- Medication management
- Usually separate from care tiers. If your parent takes daily medications — most do — treat this as part of the real monthly figure, not an extra.
- The second-person fee
- For couples sharing a residence: a flat fee for the second occupant, plus that person's own care charges if they need care. Couples with different care needs should also read each profile's couples policy line.
Every profile in our directory lists each of these separately, with its own verification date — or says plainly that the community declined to disclose it.
The two payment models in this market
Monthly rent. Most communities in this market charge month to month with no buy-in. You can leave with 30 days' notice; the trade-off is that rates rise annually and nothing about future care is prepaid.
The life-plan (CCRC) contract. A one-time entrance fee — six figures — plus a lower monthly fee, in exchange for guaranteed access to the campus's full continuum of care. Contracts differ in how much of the entrance fee is refundable and how much future care cost is absorbed; those two clauses are the whole negotiation, and a family should have an attorney read them before signing. Selling the house. Many families fund either model with a home sale. The timing problem is real — the community fee and first months' rent usually come due before the house closes — and communities are familiar with it. Ask the sales director directly how other families have bridged the gap; it is a routine conversation, not an awkward one.
The other ways families cover this
Beyond rent and entrance fees, five other sources routinely fund senior living in this market. We name each one and explain what kind of thing it is — but we do not state eligibility rules, income or asset limits, or benefit amounts here. A wrong eligibility figure on a page like this is a harm to a family making a real financial decision, not a shortcut, and we won't publish one second-hand. Every specific number belongs to the program that runs it, not to us.
- TennCare CHOICES
- Tennessee's Medicaid long-term services and supports program. It answers the question "can Medicaid pay for part of this if income and assets are limited enough." It is not automatic — it has its own medical and financial qualification process, run by the state, separate from anything on this site.

- A monthly benefit added to an existing VA pension for wartime veterans and surviving spouses who need help with daily activities. It answers "does my parent's military service unlock extra money toward care costs."

- A private policy, usually bought years in advance, that pays a daily or monthly benefit toward care once its own trigger conditions — typically needing help with a set number of daily activities — are met. It answers "does my parent already have coverage, and what does that specific policy actually pay." The answer is always in the policy document, not on this page.
- Home sale proceeds
- Using the equity from selling a house to fund the move. It answers the biggest funding question for most families here, and it is the source with no eligibility rule to verify at all — the open question is timing, not qualification.
- Bridge loans
- Short-term financing from specialty senior-living lenders, advanced against a house that hasn't sold yet or a long-term-care insurance claim that hasn't paid out yet. It answers "what do we do about the community fee and first month's rent if the house doesn't close in time."
Program rules change — TennCare CHOICES and VA Aid & Attendance both revise their financial limits over time. Confirm current numbers directly with the program before relying on them, even once this page eventually publishes figures.
Start with the communities that tell you the price
